Job Description:
Join the team at My Baby My Way Foundation, where we are redefining health and wellness for BIPOC women and underserved families. As a Community Health Worker (CHW) and/or Community Supports (CS) Worker, you will serve as a trusted advocate, educator, and connector—supporting women and families navigating a range of health, wellness, and social needs. This is a full-time, paid position for California residents only.
Your Role Will Include:
Providing culturally sensitive education on maternal, reproductive, and general family wellness
Supporting individuals in understanding birth plans, breastfeeding, postpartum care, parenting, and mental health
Referring clients to healthcare, housing, nutrition, financial assistance, and other community resources
Conducting outreach to connect Medi-Cal recipients with services, appointments, and benefits
Promoting and assisting users with MyBabyMyWay.AI, our health and wellness support app
Logging interactions accurately and following client confidentiality and care protocols
Collaborating with the team to ensure comprehensive, whole-family support
Training Provided:
All workers must complete a free Community Health Worker training provided by My Baby My Way Foundation. A certificate will be issued upon completion. Additional support and professional development opportunities are offered regularly.
Qualifications:
Passionate about community wellness, advocacy, and serving BIPOC families
Excellent communication and follow-up skills
Comfortable conducting outreach and using digital health tools
Familiarity with underserved communities and public benefits (Medi-Cal, CalFresh, etc.)
Bilingual (Spanish or other languages) preferred
Responsible, organized, and committed to compassionate care
