Community Long Term Support (CLTS) Worker

The purpose of the CLTS Worker position is to provide support and service coordination to children living with an intellectual/developmental disability, physical disability, and/or severe emotional disturbance and their family/caregivers with the primary responsibility of promoting the child’s health, safety and welfare in their home and community.

2023 Salary: $27.69 - $29.94

 Essential Duties:The following duties are normal for this position but should not be construed as exclusive or all-inclusive.

  • Assisting families to determine program functional eligibility by administering the Children’s Long-Term Care Functional Screen and assisting in financial eligibility through WI Medicaid.
  • Assessing and periodically reassessing the participant’s health, safety and functional capacity while maintain monthly contact with families through direct contact or collateral contact.
  • Providing crisis and critical incident intervention and resolution.
  • Developing Individual Service Plans with families consistent with their identified outcomes and updating the plan at the required intervals.
  • Verify that services are delivered in accordance with program requirements.
  • Assist in locating available service providers to meet outcomes identified in the service plan.
  • Providing transitional planning and support as children age into adult long-term care services.
  • Advocating and providing information for coordination with other services and resources including but not limited to: court-ordered, youth justice, or child protective services.
  • Referring and assisting families in accessing other public benefits, energy assistance, or other income based services.
  • Maintains case files and completes documentation and program paperwork within program regulations.
  • Participates in mandatory department trainings and unit trainings.
  • Participates in monthly unit meetings.
  • Performs other duties as required.

Minimum Qualifications

Qualifications may be met through a combination of education and practice experience, including:

• A course of study and practice experience that meets requirements for state certification or licensure as a social worker and also one year experience with individuals with disabilities preferred. 

 • A course of study leading to a Bachelor of Arts or Bachelor of Science degree in a health- or human services-related field and one year of experience working with individuals with 

 disabilities. 

 • A minimum of four years experience as a long-term support SSC. 

 • A combination of training and experience equivalent to four years of long-term support case management practice.

  • Working knowledge of social and economic factors which influence family life.
  • Some knowledge of principles of human behavior and development.
  • Knowledge of community resources.
  • Knowledge of public social service and assistance programs.
  • Knowledge of interviewing techniques and problem-solving methodology.
  • Have knowledge of the warning signs of child abuse and neglect.
  • Have knowledge of and ability to demonstrate an understanding of cultural competence and diversity when providing services.
  • Have knowledge of computers, computer programs and data entry.
  • Have basic knowledge of Wisconsin’s Medicaid long-term care programs, their eligibility requirements, and procedures.
  • Complete the necessary training to become certified in the administration of the Children’s Long-Term Care Functional Screen. Complete any approved and required Department of Health Services trainings.

  • Ability to communicate effectively in written and oral form.
  • Ability to maintain confidentiality and discretions regarding business-related files, reports and conversations, within the provision of open records laws.
  • Ability to apply sound judgement and in discretion in performing duties, resolving problems and interpreting policies and regulations.
  • Ability to perform detailed work accurately and independently in compliance with required timeframes.
  • Ability to establish and maintain effective working relationships with supervisors, coworkers, customers, providers and staff of other agencies.
  • Ability to manage assigned caseload and prioritize work duties.

Physical Activities/Requirements: This position will be required to perform non-sedentary and sedentary work. Non-sedentary work includes walking distances on uneven surfaces and climbing stairs; standing and sitting variable times; carrying a lap top computer and other materials simultaneously; and ability to lift up to 30 pounds. May require field visits, sometimes in buildings or terrain that is not handicapped accessible. Requirements include driving a vehicle to and from appointments. Shall maintain a valid driver's license and proof of insurance.
