Social Worker/Social Worker, Senior

THE POSITION
The Children and Family Services Coordination Center is seeking multiple Social Workers or Senior Social Workers. The Coordination Center was developed to assist in stabilizing children with their current caregivers and additionally support foster providers with new youth placements.

The Coordination Center team will seek alternative housing for youth during the times of crisis. Staff will search for and identify safe housing options, coordinate with assigned Hennepin County case workers to facilitate placements and provide a safe location for Children and Family Services staff to transition youth. 

Social Worker $57,225.27 - $71,247.95 Annually 
Senior Social Worker $58,179.28 - $83,801.71 Annually 

Location and hours:
These positions will be in a hybrid work model. Shifts may be remote online or located at the North Hub Hennepin County Human Services Center at 1001 Plymouth Avenue North, Minneapolis, MN 55411. These positions will work 8-hour shifts, Monday through Friday, between the hours of 8 a.m. and 8 p.m. Some shifts may be required to work one Saturday shift per month. Actual work shifts may vary, and candidates may give preferences. Some of the positions and shifts require staff to be physically present at the North Hub location for shifts and crisis response.

About the position type:
These are full-time, benefit-earning positions.

Starting September 1, 2021, all new job offers are conditional based on the candidate being fully vaccinated for COVID-19. “Fully vaccinated” means 14 days after receiving the second dose in a two-dose vaccination series approved by the U.S. Food and Drug Administration (FDA) on a full or emergency-use basis, or 14 days after receiving a single-dose vaccine approved by the FDA on a full or emergency-use basis. After a conditional job offer is made, the candidate must show proof of full vaccination prior to their start date. A COVID-19 vaccination card from the Center for Disease Control and Prevention (CDC), a vaccination record from your doctor’s office or a personal digital vaccination record constitutes sufficient “proof.” The candidate must also attest they are vaccinated by completing a form on the first day of employment. In these positions you will:
  • Coordinate intakes to foster homes and shelter facilities.
  • Gather youth and family information to assist case workers resolve crisis situations.
  • Work as a team, in person and remotely to find safe housing for youth.
  • Utilize multiple online data bases, manage a high volume of emails, and communicate updates to multiple parties.
  • Respond to phone inquiries and make phone contact with multiple housing providers.
  • Maintain and develop emergency placement resources, homes, and facilities.
  • Provide and facilitate the foster family receiving as much information as possible to best meet the needs of children in their care.
  • Support foster parents, children, and families to prevent disruption. 
  • Support Quality Parenting Initiative (QPI) practices at the time of placement focusing on building a co-parenting relationship between the foster parent and birth parent(s) to discuss vital information needed to meet the child’s needs.
  • Train investigations, ongoing, induction, foster care licensing, and case management staff on the QPI and placement support team tasks.
Need to have:
  • For hire as a Social Worker, one of the following:
  • Bachelor's degree in social work, psychology, sociology, human services, or equivalent field
  • Licensed as a Social Worker by the Minnesota Board of Social Work.
  • For hire as a Senior Social Worker, one of the following:
  • Master's degree or higher in social work or closely related field with course work emphasis in areas such as social work methods, human growth and behavior, and principles or social welfare.
  • Bachelor's degree in social work, psychology, sociology, or human services, and at least 27 quarter credits of graduate work in the above areas, and three years or more of full-time supervised experience as a social worker.
  • Licensed as a Graduate Social Worker by the Minnesota Board of Social Work.
  • A valid driver's license and daily access to reliable transportation to travel primarily within the metro area to meet with persons in their residential settings and/or perform authorized off-site work duties. 
Nice to have:
  • Current State of Minnesota Certificates/Licensures. 
  • Experience:
  • Facilitating youth placements in the social service system.
  • Documenting tasks completed toward placement or diversion.
  • Finding temporary out of home placement resources; including monitoring of placement options/community home resources.
  • Utilizing data systems such as Social Service Information Systems (SSIS), Enterprise Communication Framework (ECF), Minnesota Court Information System (MNCIS), Minnesota Government Access (MGA), Placement Request System, Power-BI, and Microsoft Office, Outlook, and Teams.
  • Working in a hybrid work environment to accommodate remote online work and on-site work.
  • Ability to:
  • Coordinate and communicate effectively with staff to effectively meet the needs of youth.
  • Monitoring and providing support for youth on-site.
  • Work with diverse communities.
