Family Nurse Practitioner/Physician Assistant

THE POSITION
Join us as we change the way health care is provided to those who need us most. Hennepin County Health Care for the Homeless is seeking a part-time Family Nurse Practitioner or Physician Assistant who has an interest in and skill and experience working with individuals experiencing homelessness. In this role, you will provide health care services at local shelters and drop-in centers in the downtown Minneapolis area. Preventative and acute care services are provided on a walk-in basis to adult men and women, youth, children, and infants. You'll join a strong, diverse, multidisciplinary team of medical providers, registered nurses, social workers, licensed practical nurses and medical assistants.

Location and hours:
This position is located in clinics at shelters and drop-in centers throughout downtown Minneapolis and will require travel between the different locations. Regular work hours will likely include one evening per week in addition to picking up shifts, as necessary, to back up providers who are out, totaling up to 32 hours per week. Clinic hours range from 8 a.m. to 8:30 p.m. No weekend, overnight, or holiday shifts are required.

Nurse Practitioner $33.04 - $69.20 Hourly
Physician Assistant $45.24 - $69.20 Hourly 
 
About the position type:
This is a part-time (32 hours/week), benefit earning position.

Starting September 1, 2021, all new job offers are conditional based on the candidate being fully vaccinated for COVID-19. “Fully vaccinated” means 14 days after receiving the second dose in a two-dose vaccination series approved by the U.S. Food and Drug Administration (FDA) on a full or emergency-use basis, or 14 days after receiving a single-dose vaccine approved by the FDA on a full or emergency-use basis. After a conditional job offer is made, the candidate must show proof of full vaccination prior to their start date. A COVID-19 vaccination card from the Center for Disease Control and Prevention (CDC), a vaccination record from your doctor’s office or a personal digital vaccination record constitutes sufficient “proof.” The candidate must also attest they are vaccinated by completing a form on the first day of employment. Reasonable accommodations to this vaccination requirement will be considered upon request by a candidate for medical or religious reasons as required by applicable law. 

In this position, you will:   
  • Perform initial and ongoing assessments of patient medical, physical, and psychosocial status, while doing triage for patients who require a higher level of care.
  • Obtain and evaluate patient health and medical history and document in the EPIC Electronic Health Record (EHR).
  • Perform physical examinations and conduct preventative screening procedures based on age and medical history; identify medical and health risks.
  • Complete accurate and timely documentation of observations, interventions, referral, and follow-up; develop patient care plan.
  • Provide health education, counseling, consultation, and advocacy based on needs identified in care plan.
Need to have:
  • For hire as a Nurse Practitioner:
  • Graduation from an accredited school of nursing.
  • Possession of a valid license as a Registered Nurse issued by the state of Minnesota and a National Certification as a Nurse Practitioner (CNP), Certification in the area of specialization by an agency authorized to certify professional nurses in advanced practice OR an approved equivalent as provided under M.S. 148/284.
  • Fore hire as a Physician Assistant:
  • Graduation from an accredited physician assistant (PA) program.
  • Certification or eligibility for certification from the State of Minnesota as a Physician Assistant. Must also complete the Delegation Agreement and Notice of Intent to Practice as required by the MN Board of Medical Practice.
  • Access to reliable transportation to travel among shelters and drop-in centers to provide medical services.
Nice to have: 
  • One or more years of full-time experience working as a Family Nurse Practitioner or Physician Assistant.
  • Current CPR certification.
  • An X Wavier and the ability to prescribe buprenorphine.
  • Experience:
  • Providing healthcare services to people experiencing homelessness.
  • Working with Electronic Health Records (EHR).
  • Sensitivity to the needs and issues of diverse and disenfranchised individuals.
  • Strong leadership and written and verbal communication skills.
  • Ability to:
  • Work independently and interdependently with professionals from other disciplines.
  • Establish and maintain effective relationships with internal and external partners.
