This posting represents two distinct case management roles:

Position 1: Case Manager – Adult Medical Day-Case Manager

Supports Planners identify, coordinate, and monitor waiver services that promote health, safety, independence, and quality of life. This role includes waiver eligibility determination and ongoing maintenance.

Salary & Bonus:

• Base Salary: $60,000
• A bonus potential of $1,250 per quarter, contingent upon meeting performance requirements.

Key Responsibilities:

• Build person-centered relationships.
• Develop and maintain Plans of Service.
• Educate individuals on service options.
• Coordinate providers and monitor service delivery.
• Conducting home visits.
• Maintain waiver eligibility and documentation.

Requirements:

• Bachelor’s degree in human services or related field (or approved equivalency) or a foreign degree with an educational evaluation from an accredited agency.
• Supports Planner: Some case management experience.
• Coordinator of Community Services: Minimum 2 years of case management experience.
• Valid driver’s license, reliable transportation, and safe driving record
• Strong communication, organization, and time-management skills
• Must be at least 21 years old
• Legally authorized to work in the U.S.
• Ability to pass background checks
