About the Role
Pearl Healthcare is looking for a Social Service Director to join our dedicated team. In this role, you'll lead and coordinate social services for our residents and patients, ensuring they receive the support and resources they need to thrive. Social services at Pearl Healthcare isn't a back-office function — it's central to the care we deliver every day.
What you'll do
- Assess residents' social, emotional, and psychological needs and develop individualized care plans- Coordinate with interdisciplinary teams — nursing, therapy, dietary, and administration — to support residents' overall well-being- Connect residents and families with community resources, financial assistance programs, and support services- Manage discharge planning and transitions of care, working closely with hospitals, families, and community agencies- Maintain accurate and timely documentation in compliance with federal, state, and facility regulations- Advocate for residents' rights and dignity throughout their stay- Lead and supervise social services staff where applicable- Participate in quality assurance and facility-wide care planning meetings
About you
- Degree in Social Work, Human Services, Psychology, or a related field (BSW or MSW preferred)- Previous experience in a long-term care, skilled nursing, or healthcare setting preferred- Knowledge of federal and state regulations governing social services in long-term care- Strong communication and interpersonal skills — you can work with residents, families, and clinical staff with equal ease- Organized, detail-oriented, and comfortable managing multiple priorities- Genuine commitment to resident well-being and advocacy
