The Grove Post-Acute is a 140-bed premier post-acute community in Woodland, California, located just about 30 minutes from Midtown Sacramento. We are committed to providing exceptional outcomes and compassionate, resident-centered care in a supportive community environment.

We're looking for a dedicated Occupational Therapist (OTR/L) to help residents improve their independence, regain functional abilities, and achieve their rehabilitation goals.

What You'll Do:Evaluate residents and develop individualized treatment plansProvide high-quality occupational therapy servicesEstablish treatment goals based on residents' functional needsEducate residents, families, and staff on treatment plans and progressCollaborate with an interdisciplinary healthcare teamMaintain accurate documentation, progress notes, and EMR recordsPromote safe, effective, and resident-centered careMake appropriate referrals to other members of the rehabilitation teamParticipate in quality assurance and continuing education activities

Qualifications

Graduate of an approved Occupational Therapy programActive, unencumbered California Occupational Therapist licenseCurrent CPR certification, as requiredWorking knowledge of Medicare and other payer sourcesStrong communication, organization, and teamwork skillsAbility to manage multiple tasks effectivelyExperience in skilled nursing or long-term care preferred
