Community Outreach & Client Relations Representative

Position Summary

Better Choice Home Health seeks a motivated, personable, community-focused individual to help grow our agency by building relationships with healthcare professionals, community organizations, patients, and families throughout the Rio Grande Valley.

This position will focus on bringing new clients to Better Choice Home Health, educating patients and families about home health services, and building strong referral relationships throughout our service area.

Key Responsibilities

• Develop relationships with physicians, hospitals, clinics, rehabilitation centers, nursing facilities, assisted living communities, social workers, and other healthcare professionals.
   
• Identify individuals and families who may benefit from home health services and help connect them with Better Choice Home Health.
   
• Educate patients, families, and community members about our home health services and how home health can support recovery and independence at home.
   
• Conduct regular community and referral-source visits throughout the Rio Grande Valley.
   
• Represent Better Choice Home Health at health fairs, community events, senior programs, and networking events.
   
• Follow up with potential clients and referral sources in a professional and timely manner.
   
• Maintain positive, long-term relationships with referral partners.
   
• Communicate community needs, referral opportunities, and outreach activities to agency leadership.
   
• Maintain accurate records of outreach and referral activities.

• Friendly, professional, and comfortable speaking with patients, families, and healthcare professionals.
   
• Self-motivated and able to work independently.
   
• Strong networking and relationship-building abilities.
   
• Healthcare, home health, sales, marketing, or community outreach experience preferred.
   
• Bilingual English/Spanish is required.
   
• Must be willing to travel throughout the Rio Grande Valley.
