Case Manager (Homeless Services)
Agency: Reach for Home (RFH) Job Title: Case Manager Reports to: Director of Housing Hours: 40 hrs/week, Non-Exempt Pay Range: $26-29/hour Job Description Reporting to the Director of Housing, the Case Manager (CM) will provide varying levels of case management, crisis intervention, and support services to participants in Reach for Home's housing programs. The CM is responsible for completing client intake/assessment to identify needs and strengths of participants for services, as well as providing program information and referrals of services and resources.
Essential Duties
- Guide participants through development and implementation of Independent Action Plans and budgets to meet the participants' short and long-term needs and goals following Housing First principles. Monitor adherence to plans.
- Provides crisis management for clients; makes linkages for interventions as appropriate
- Facilitates life skills groups in conjunction with the Lead Case Manager and Director of Programs Determine appropriate financial assistance and credit counseling
- Provide thorough, timely, and complete documentation of all required activities and engagements, including case notes, Reasonable Accommodation requests, HUD Entry/Exit/Annual Assessments, etc. on HMIS. Ensure that participant files are maintained and up to date according to grant and program contracts.
- Act as an advocate for participants to ensure access to appropriate community resources to increase independence, i.e. connections to employment or income opportunities as well as other resources that will elevate their situation.
- Build ongoing trust with participants.
- Ability to work effectively with individuals from diverse backgrounds and cultures, and awareness of and sensitivity to the cultures represented in Sonoma County
- Assist and support participants with housing searches and identify housing barriers.
- Facilitate and maintain community relationships to create referral pathways for program participants. Participate in agency program meetings as appropriate.
- Deliver services following Trauma Informed Care principles and Best Practices outlined by the agency. Ensure that RFH housing sites have proper maintenance and report all issues to property manager or supervisor immediately.
- Participate in agency safety initiatives and proactively report safety issues to management immediately.
- Other duties as assigned.
Qualifications/Skills:
- High level skills in communication (written and oral)
- Deescalation
- Experience navigating client care systems
- Experience working with data bases
- Interpersonal skills
- Trauma Informed Care trained preferred but not required
- Bilingual (Sp/Eng) preferred but not required
