About the Role

Ascend Mental Health and Wellness is a growing private practice in Hampton Bays that offers psychiatric medication management and psychotherapy under one roof. We're looking for experienced, compassionate LCSWs and LMSWs who want to get in on the ground floor of something new — a collaborative, clinician-friendly environment where you work alongside other therapists and our psychiatric nurse practitioner to deliver genuinely integrated mental health care.

This isn't a mill. You'll have real relationships with your clients, real collaboration with colleagues, and real input into how the practice grows.

What you'll do

- Carry an individual caseload of therapy clients across a range of presenting concerns — anxiety, depression, trauma, life transitions, and more.- Coordinate closely with our psychiatric nurse practitioner on shared clients to ensure medication management and therapy are working together.- Conduct intake assessments and develop individualized treatment plans.- Maintain accurate, timely clinical documentation in the EHR.- Participate in regular clinical consultation and peer collaboration.- Flexible scheduling — you set your hours within the 10–40 hr/week range.

What we're looking for

- Active New York State LCSW or LMSW licensure (required).- At least 2 years of direct clinical experience providing individual therapy.- Solid grounding in at least one evidence-based modality (CBT, DBT, psychodynamic, EMDR, or similar).- Strong clinical documentation habits.- Genuine interest in collaborative, integrated care.

Nice to have

- Experience working with psychiatric populations or alongside prescribers.- Familiarity with billing and insurance credentialing processes.- Telehealth experience.
