Location: Southern Orange County, CA | Onsite | Full-TimeSalary: $80,000–$85,000/year + 15% bonus potential
The role
We're looking for a Social Services Director to join a 5-star, 49-bed skilled nursing facility in southern Orange County. You'll own the social services function — managing how residents navigate post-acute care, staying connected with their families, and making sure the care planning process actually runs the way it's supposed to.
What you'll do
- Manage post-acute case workflows for residents across the full continuum of their stay.- Coordinate care plans alongside residents, family members, and facility staff — making sure everyone is on the same page.- Navigate regulatory requirements including Title 22, OBRA, Medicare, PPS, MDS, and HIPAA without losing sight of the person at the center of it all.- Serve as the primary social services point of contact for residents and families, handling concerns, transitions, and discharge planning.- Maintain accurate, timely documentation in compliance with facility and regulatory standards.
About you
Required:- Degree in Social Services with 5+ years of experience in a skilled nursing or post-acute setting, OR an LVN license with 5+ years of relevant experience.- Working knowledge of Title 22, OBRA, Medicare, PPS, MDS, care planning processes, and HIPAA.
Nice to have:- Prior director-level or lead role in a SNF environment.- Experience with electronic health records and MDS coordination.
What we offer
