Position Overview
This position supports the agency's efforts in providing comprehensive, compassionate social work services for older adults, their families, and significant others. These services include a combination of psychosocial assessments, person-centered care plan interventions, caregiver support, pro-active planning for life changes, and opportunities for education via workshops, trainings, and conferences. Provides social work services to seniors and family members who serve as caregivers.
Essential Job Functions
- Lead community education for older adults and/or their caregivers in the form of workshops, Town Halls, and/or conferences.
- Provide support services in areas of social work case management and information and assistance lines. Identify need for referrals both internally and externally. Identify mechanism for follow up for monitoring of continued need.
- Conduct both brief and comprehensive inclusive, geriatric, psychosocial assessments of clients' needs (in the office or during a home visit) for emotional and social support, community resources, information, guidance, intervention and client advocacy. Clients may include low-income or fee for service clients.
- Develop action plans for case management clients, identifying interventions and follow up. Monitor outcomes of interventions to ensure client needs are met through client satisfaction surveys and enhanced wellness, emotional and or social outcomes such as increased socialization and decreased social isolation.
- Maintain accurate records of client contacts through electronic chart documentation, charting all client contact, referrals made, and other work on behalf of clients within 48 hours of service.
- Social Worker will participate in outreach and learn area resources to connect case management clients and information and assistance callers to needed resources.
- Cultivate community partnerships for cross-referral, awareness, and educational purposes.
- Secure budget approval from ASWS supervisor for all related expenses for client emergency or mental fund assistance.
- Provide monthly statistical data to ASWS supervisor by the sixth day of the month following services. Stats need to reflect the work done the prior month for accuracy of reporting to funding sources and Board.
- Attend weekly supervisory sessions for clinical and administration consultation and agency meetings as needed for guidance on areas of responsibility.
Qualifications
Education and Experience
- Bachelor of Social Work (BSW) with 2 years care management experience strongly preferred.
- Candidates with a degree in another relevant social services field, such as Psychology, may be considered with equivalent experience.
Knowledge, Abilities and Skills
- Knowledge of and capacity to utilize community resources; ability to assess physical and social needs of frail older adults.
- Knowledge of Medi-Cal systems and programs.
- Highly organized and able to manage time wisely.
- Strong oral and written communication skills.
- Must be willing and able to perform duties related to public speaking.
- Knowledge of working with people from diverse backgrounds, willingness to learn.
- Experience with older adults and caregivers
- Ability to work well with other department team members to support clients.
- Travel to contracted agencies for care management support
- Experience with Electronic Health Records
- Must possess own transportation and insurance to complete necessary in-home visits (mileage reimbursed).
Physical Demands and Work Environment
- Frequent stand/walk, sit, use a computer; use a telephone, grasp lightly/fine manipulation.
- Occasionally reach/work above shoulders, twist/bend/stoop/squat, grasp forcefully, lift/carry/push/pull objects that weigh up to 10 pounds, write by hand, sort/file paperwork.
Hours and Compensation
This is a full-time, non-exempt, 40-hour per week position. Benefits include medical, dental, vision, 403B grant offered; paid vacation, sick leave, holidays and more. The current pay range is $33.52 - $37.25 per hour.
