Job Overview

Care Associates is seeking a compassionate and highly organized Social Worker to support our integrated service lines and participation in the CMS Guiding an Improved Dementia Experience (GUIDE) Model and our broader medical practice.

This is a patient- and caregiver-facing position for a social worker who enjoys helping older adults and families navigate complex healthcare needs.

You will serve as a primary point of contact for Care Associates patients and their caregivers, helping families move from referral and enrollment to the service line(s) available, to ongoing care coordination, caregiver support, community-resource navigation, and interdisciplinary care planning.

Duties

• Guide patients and caregivers across Care Associates service lines and manage referrals, intake, enrollment, and ongoing customer support activities.
• Conduct phone outreach, care support follow-up, and GUIDE-prescribed patient/caregiver check-ins.
• Assist with standardized assessments, onboarding documentation, and person-centered care plans.
• Identify caregiver stress, psychosocial concerns, safety issues, and unmet needs in through GUIDE care planning activities; help coordinate appropriate support and monitor outcomes.
• Connect families with community resources, respite services, caregiver education, and other available services.
• Manage inbound and outbound documents in compliance with HIPAA and maintain accurate and timely document workflows within the electronic health record (EHR).
• Track contacts, assessments, timelines, and other required activities across applicable service lines and the GUIDE Program.
• Assist with scheduling, reports, correspondence, and quality and compliance activities.
• Participate in occasional local home or community visits.
• Participate in weekly leadership strategy meetings.

Qualifications

• Bachelor’s or Master’s degree in Social Work or a related field with relevant licensing or certification preferred.
• Strong communication skills—both written and verbal—to effectively advocate for clients and collaborate with multidisciplinary teams.
• Proficiency in Microsoft Office applications for documentation and reporting purposes.
• Experience working in medical settings, case management, care coordination, or care navigation.
• Experience working with older adults, dementia, serious illness, or family caregivers.
• Familiarity with Medicare, CMS programs, value-based care, or standardized clinical assessments.

Pay: $48,000.00 - $55,000.00 per year
