MISSION/VISION 

Healthcare Alternative Systems (HAS) provides a continuum of multicultural and bilingual (English/Spanish) behavioral care and social services that empower individuals, families, and communities. HAS seeks to implement sustainable programs that benefit society by improving the well-being and recovery efforts of those individuals, families, and communities impacted by behavioral health problems. With eleven locations across the Chicagoland area, HAS provides services and resources to individuals at every stage in their behavioral/recovery journey so that they can be successful and become contributing members of their communities.
 

JOB SUMMARY 

The Community Support Team (CST) Mental Health Lead is charged with the responsibility for direct supervision of their team and coordination of culturally competent, trauma informed and client centered services to the participants enrolled into the program.  The team lead will be responsible for building a cohesive team and a successful program through coaching, mentoring and monitoring of productivity of staff.  The Lead will be expected to regularly interact with community partners in building referral relationships along with internal HAS leadership. The lead may also provide direct interventions with persons served including psychotherapy, counseling, and coordination/development of the person-centered plan.

 

SUPERVISORY RESPONSIBILITIES

Provides evaluative and coaching feedback to providers under supervision according to policy.

Provides clinical guidance, supervision and training to the CST members in the team’s interventions with the recipient using client-centered motivational interviewing, harm reduction, DBT, and other modalities that are used with individuals with dual diagnoses.

Provides the clinical supervision of all members of the team for the provision of this service. An individual supervision plan is required for all CST members.

Ensures compliance with evidence-based practice. Provides detailed reports as required.

Participates in ongoing Quality Improvement activities and internal/external trainings.

Demonstrates ability to provide effective administrative and clinical supervision.

Performs other tasks as assigned.

ESSENTIAL DUTIES 

 

Maintains a small caseload and provides individual recovery-based therapy for recipients served by the team.

Designates the appropriate team staff to provide needed interventions so that specialized expertise is applied as clinically indicated for each participant.

Completes and signs all documentation, when necessary, per Rule 140 requirements, including IMCANS and Addendums, and ensure chart compliance.

Provides and coordinates the assessment and reassessment of the participant’s clinical needs.

Demonstrates knowledge of adult mental illness and treatment modalities for working with individuals with chronic serious mental illness. Skill in the use of motivational interviewing, harm reduction, and engaging Participants in self-directed care.

Ensures that care provided integrates all facets of Participants lives including mental/emotional health, physical health/wellness, employment and education, social and family relationships, substance use/abuse issues, and housing needs.

Assess Participants personal, medical, social, emotional and their environmental situation through home, community and office visits.

Assist Participants in identifying signs and symptoms of de-compensation and the need for either a higher level of care or hospitalization.  Work collaboratively with the Participants to identify their coping skills to manage symptoms and stressors

Support fellow team members with crisis interventions and wellness checks.

Support team members in working to assist Participants in learning and improving their independent living skills, applying for entitlements, accompanying Participants to important appointments in the community and providing support with issues related to housing, substance use, budgeting, medical issues, social support, and medication.

Manages referrals and determines team caseload by the level of acuity and the needs of the individual served.

Facilitates daily CST team meetings to coordinate care, assignments and issues.

Coordinates and participates in 24/7 on-call scheduling.

Monitors staffing needs and participate in hiring and orientation of new staff.

Familiarity on completing authorizations for services for Insurance/MCO’s

Qualifications

 

REQUIRED SKILLS/ABILITIES 

Demonstrates high moral and ethical character, and be free of any substantiated charges of abuse or neglect on the Healthcare Registry.

Experience in community-based services preferred.

Possession of valid State Driver’s License and willingness to use personal vehicle in course of employment.

HAS requires all staff to work onsite.

EDUCATION & EXPERIENCE 

Bachelor’s Degree in Social Work or related field, from a regionally accredited educational institution which has U.S. Department of Education approval. Master’s degree is preferred.

Fully Licensed (LPHA) mental health professional with a LCSW or LCPC is preferred. Qualified Professional (QMHP) or LSW, LPC actively seeking licensure may be considered and position is conditional upon being fully licensed within 3 months from the effective date of hire.

Have one (1) year documented clinical supervisory experience.

Have three (3) years documented direct experience with people with mental health and/or substance use diagnoses or other relevant populations that will be served.

IM-CANS certified or obtained within 30 days of hire.

PHYSICAL REQUIREMENTS 

Sedentary work. Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Repetitive motion. Substantial movements (motions) of the wrists, hands, and/or fingers. The worker is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing, viewing a computer terminal, and extensive reading.
