MISSION/VISION
Healthcare Alternative Systems (HAS) provides a continuum of multicultural and bilingual (English/Spanish) behavioral care and social services that empower individuals, families, and communities. HAS seeks to implement sustainable programs that benefit society by improving the well-being and recovery efforts of those individuals, families, and communities impacted by behavioral health problems. With eleven locations across the Chicagoland area, HAS provides services and resources to individuals at every stage in their behavioral/recovery journey so that they can be successful and become contributing members of their communities.
JOB SUMMARY
Under the direct supervision of the Program Manager, the Domestic Violence Case Manager is responsible for offering technical assistance and case management linkage and referrals as needed. Additionally, the Domestic Violence Case Manager coordinates with the treatment team the plan of care for participants with mental illness and/or substance use issues. A Domestic Violence Case Manager acts as a liaison between participant/family and clinician to ensure that their non-clinical case management needs are being met.
ESSENTIAL DUTIES
- This is a Part-Time position
Manages a caseload of participants to ensure adequate services are being provided to assist participants with meeting their treatment needs
Participates in regular staff meetings held at position location.
Links participants to needed and available community resources including legal, medical, housing, transportation, mental health, financial, etc.
Provides ongoing supportive and/or case management functions in accordance with the problems/needs of the participant, and the strategies identified within the service/treatment plan in order to help the participant to achieve the stated goals and objectives.
Provides face-to-face reviews with the participant and/or relevant staff on a regular basis.
Documents all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with agency and regulatory standards and requirements.
Collaborates with the treatment team to ensure compliance and to work toward improved participant outcomes.
Provides anti-stigma education to the community and individuals dealing with mental illness and/or substance use disorders as well as information on prevention, treatment, and recovery for participants.
Meets regularly with the Program Manager as a means of enhancing professional growth, reviewing and processing the provision of case management services, and dealing with appropriate administrative issues.
Maintains a networking relationship with community agencies, clinics and supportive services for referrals/linkages and ancillary services.
Provides specific case management support, services, and documentation associated with any private or public grants.
Provides monthly progress reports to Program Manager.
Performs other duties as assigned by the Program Manager.
Qualifications
REQUIRED SKILLS/ABILITIES
Must be willing to travel locally and will require work to be done on-site.
Strong organizational, communication, and critical thinking skills, including the ability to work as part of a collaborative treatment team.
Basic knowledge of computer keyboard.
Bilingual/Bicultural (English/Spanish), preferred.
Must be flexible with hours to accommodate participant needs.
Possession of valid state Driver’s License and willingness to use personal vehicle in course of employment, including transporting program participants as needed.
EDUCATION & EXPERIENCE
Bachelor’s degree in a human services field (i.e., social work, psychology or sociology) from a regionally accredited educational institution which has U.S. Department of Education approval, preferred; or a minimum of two (2) years in college working on degree, required.
Two (2) years of case management experience in a non-profit setting, preferred.
Completed 40-hour training in Domestic Violence and completed the additional 20 hours of abuser service training, or able to complete within 3 months of employment.
PHYSICAL REQUIREMENTS
Sedentary work. Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Repetitive motion. Substantial movements (motions) of the wrists, hands, and/or fingers. The worker is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing, viewing a computer terminal, and extensive reading.
