Are you interested in working in the mental health field and working with adults who are homeless? CHCS currently has an opening for a Navigator on our Maine Projects for Assistance in Transition from Homelessness Team. The PATH Navigator works as a member of a team of professionals who provide outreach, engagement, case management and service coordination services to adults, who are homeless and have a mental illness or co-occurring mental illness and substance use disorder. These individuals are eligible under the Maine Projects for Assistance in Transition from Homelessness (PATH) Program. Will be responsible for the identification of eligible individuals, assessment of needs, planning, linking to resources, monitoring and evaluation of services and support needed by homeless individuals. 

 

Must have an associate degree in a human service field with three (3) years relevant experience or a bachelor’s degree in a human service field with one (1) year of relevant experience. Must minimally meet MHRT/C provisional certification requirements at time of hire and be able to complete the MHRT/C certification requirements within two years of employment. Must have two years of experience engaging homeless people with disabilities. Must have the ability to participate in street outreach, engage people who are homeless, perhaps living outdoors, at shelters, or other community locations. Must have a valid driver’s license and a minimum of $100,000/300,000 liability insurance.
