A Day in the Life
The Coord Patient Home Care provides oversight for a patient’s clinical care, outcomes, and service satisfaction. The position will provide field staff leadership and support ensuring care provision follows evidence-based practice and is delivered in the most efficient and fiscally responsible manner.
What’s Required
The employee must be a graduate of an approved school of Nursing or Physical Therapy.

Must hold a current licensed as Registered Nurse or Physical Therapist in the state of Michigan and in good standing.
For those hired into the position prior to January 1, 2015 with a Nursing degree, it is encouraged to obtain a BSN or related BA/BS. Certification in the specialty area of practice is also encouraged.
For those hired into the position after January 1st, 2015 with a Nursing degree, it is required to obtain a BSN or related BA/BS within 3 years of start date.
- 3 years of clinical experience with two 2 years of Home Care or Hospice experience required.
- Meets federal and state home health regulation by passing a criminal background check and fingerprinting.
- Possesses a current Michigan motor vehicle license, the ability to drive a car, and possess an insured reliable vehicle that can be driven to offices/patient homes in various weather conditions.
- Meets MHH established requirement for proof of health status to include vaccinations. Physically able to lift 35 pounds. Demonstrates ability to move, position and transfer clients utilizing good body mechanics, lifting techniques and/or use of transfer assistive devices.
- Possess knowledge of the Conditions of Participation (CoP), accreditation standards and Medicare, Medicaid, and commercial insurance reimbursement requirements.
- Home Care Specific: OASIS certification preferred. Will be required to obtain within 1 year of hire.
- Required to be self-directed professional with ability to communicate professionally and effectively orally and in writing.
- Required to be computer literate including efficient email management, use of virtual meeting platforms and Microsoft word, excel and power point.
