Salary: $23.30/hour - Salary is Negotiable (Employees will receive 5% below the negotiated pay rate during their probationary status.)

Responsible To: Case Management Program Manager

Summary: This Social Work position supports patients and their families as they navigate the emotional, social, and practical challenges associated with illness, nursing home and ongoing long term care medical needs. They work as part of an interdisciplinary healthcare team to ensure patients receive holistic, patient-centered support. This position, as part of an interdisciplinary team (IDT), serves frail elders, adults with physical disabilities and adults with intellectual/developmental disabilities who are patients of the Oneida Community Health Center (OCHC). The goal of the position is to provide high quality, person-centered, outcome-based care. Continuation of this position is contingent upon funding allocations.

• Bachelor’s Degree in Social Work. 
• Must be licensed as a Social Worker by the Wisconsin Department of Safety and Professional Services.
