The High Fidelity Wraparound (HFW) Facilitator serves as the operational hub of the Child and Family Team (CFT). In this vital role, you will guide families and their teams through all four phases of the Wraparound process, ensuring that every decision reflects the family's voice, strengths, culture, and vision. You'll facilitate Child and Family Team meetings, complete the Strengths, Needs, Culture, and Vision (SNCV) Discovery, develop and update individualized Plans of Care, coordinate services, and monitor progress toward meaningful goals. Following each meeting, you'll ensure that team decisions are translated into timely, actionable interventions that support positive outcomes.

In This Role, You Will
- Partner with children, youth, and families to develop individualized, strengths-based plans
- Facilitate Child and Family Team (CFT) meetings
- Complete the SNCV Discovery process
- Develop, implement, and update High Fidelity Wraparound Plans of Care
- Coordinate services among families, providers, schools, and community partners
- Advocate for youth and families while promoting family voice and choice
- Monitor progress toward goals and adjust plans as needed
- Build strong partnerships that promote sustainable, positive outcomes
Required Qualifications
- Bachelor's or Master's degree in Social Work, Psychology, Counseling, Human Services, Education, or a related field
- Minimum of two years of experience working in behavioral health, social services, education, child welfare, or a related field
- Experience coordinating multidisciplinary teams or facilitating collaborative planning meetings
- Strong organizational, communication, and facilitation skills
- Demonstrated experience using family-centered, strengths-based practices
- Commitment to providing trauma-informed, culturally responsive services
- Passion for empowering children, youth, and families to achieve their goals
