Care Manager Extender / CPSS -- Your Medicaid Navigation and Tailored Care Management Skills Matter Here (Full Time)

You’ll be a key partner in helping people build confidence, develop skills, and connect to resources that support their success.

 

What You’ll Do

As a Care Manager Extender, you will:

  • Conduct outreach, engagement, and follow-up with the individuals we support
  • Partner closely with Care Managers to support person-centered goals and plans
  • Help individuals strengthen skills related to:
    • Independent living (housing, finances, employment readiness)
    • Communication and relationship-building
    • Self-advocacy and boundary setting
    • Community connection and social supports
  • Provide guidance on navigating systems and accessing resources
  • Support individuals in preparing for conversations with employers, landlords, and service providers
  • Assist with pre-crisis support and promote strategies that improve overall well-being and safety
  • Encourage healthy lifestyle choices and support individuals managing chronic conditions

 

What We’re Looking For

To qualify, candidates must have:

  • Must be at least 18 years of age
  • High School Diploma or equivalent
  • North Carolina Certified Peer Support Specialist (CPSS) credential
  • At least 2 years of direct experience supporting individuals and navigating the Medicaid system
  • At least 2 years of paid experience performing similar responsibilities
  • Minimum of 1 year working directly with the Tailored Care Management eligible population

 

AND meet one of the following:

  • Have lived experience with a behavioral health condition
    OR
  • Be a parent/guardian of an individual with a behavioral health condition with at least 2 years of relevant experience
    (Note: parent/guardian cannot serve as an extender for their own family member)
