Duties

  • Conduct comprehensive patient assessments to determine physical therapy needs, utilizing your knowledge of anatomy, physiology, and medical terminology.
  • Develop and implement individualized care plans focused on recovery goals for patients across pediatrics, geriatrics, orthopedics, and other specialties.
  • Deliver hands-on therapies such as trigger point therapy and other physiotherapy techniques to alleviate pain and restore function.
  • Coordinate discharge planning with multidisciplinary teams to ensure seamless transition from hospital to home or community settings.
  • Manage patient progress through regular evaluations, adjusting treatment plans as necessary to optimize outcomes.
  • Document all patient interactions accurately within electronic medical record (EMR) systems while adhering to compliance standards.
  • Provide education to patients and families on exercises, injury prevention, and health maintenance strategies for home & community care.

Skills

  • Strong knowledge of anatomy, physiology, and medical terminology essential for accurate assessment and treatment planning.
  • Proven experience in inpatient hospital settings, outpatient clinics, home health services or community care environments.
  • Expertise in discharge planning, care plan development, and coordination with healthcare teams.
  • Skilled in various physiotherapy techniques including trigger point therapy tailored to individual patient needs.
  • Familiarity with EMR systems for efficient documentation and patient record management.
  • Ability to work effectively with diverse populations including pediatrics and geriatrics in both hospital and home & community care settings.
