We are conducting interviews for the following positions:
PACE Personal Care Aide:
Position Summary:
Under the supervision of the PACE RN Supervisor and the day center nursing team, the Personal Care Aides & Certified Nursing Assistants provide support in the delivery of personal care to participants and to participants in need of companionship services to and from medical appointments. Services include, but are not limited to, participant care activities such as showering, toileting, serving, feeding and other Activities of Daily Living. Position will assist with home delivery of meals and medical supplies. Position may also assist in other areas such as kitchen, primary care and therapy.
Job Requirements:
- Six (6) months experience as a personal care aide, in a professional or personal care setting
- High School Diploma, GED or six months experience as a personal care aide, in a professional or personal care setting.
PACE Certified Nursing Assistant
Position Summary:
Provides direct patient care services to the participants under the direction and supervision of a nursing staff member, as assigned by the Program Director.
Job Requirements:
- High School Diploma, GED or one year experience as a CNA.
- Nurse’s Assistant California State Certification
- Current CPR Certification/Card
PACE Home Care Coordinator
Position Summary:
As a collaborative member of the IDT, and under the supervision of the PACE Home Care Coordination RN Manager, the PACE C.N.A with Home Care Coordinator HHA Requirement provides foundational support to the Home Care Coordination team. This role conducts home safety evaluations, coordinates and monitors IDT approved services, such as the delivery of meals, caregiver services and DME (durable medical equipment) for San Diego PACE participants. The Home Care Coordinator HHA is an advocate for participants' healthcare needs and preferences, ensuring that care is delivered in a person-centered and compassionate manner; working closely with participants, their families, and the interdisciplinary team to ensure that all aspects of unskilled home- based care is coordinated and aligned with the participants' goals and preferences. They liaise with, care giver agencies, DME suppliers, etc. to coordinate schedules, monitor the delivery and quality of services/care provided, and make recommendations for adjustments to care plans as needed.
Job Requirements
- CNA certificate
- Must obtain Home Health Aide certificate within 120 days of employment
PACE Outreach Specialist
Position Summary:
The Outreach Specialist will be primarily responsible for conducting outreach in the community to raise awareness of and promote the PACE program. The Outreach Specialist will participate in events and fairs within the community. This position will work collaboratively with the Enrollment Specialists with the goal of increasing census within the program.
Job Requirements
- High School Diploma or GED
- At least 2 years’ experience working in a customer service role
- CA valid driver’s license and reliable transportation
