Social Worker Assistant
The Social Worker conducts various assessments, develops plans of care, and coordinates social services with participants, family members, physicians, center staff, and community agencies.
The duties include:
- Interview applicants, conduct various assessments, and develop plan of care for the participants
- Monitor and update the plan of care as needed and/or desired by the participants.
- Maintain the social services component of the participant health records on a regular basis.
- Coordinate with other community agencies to provide services to participants and provide referrals as needed.
- Provide a treatment program to participants, including counseling and group discussions to address identified problems.
- Lead participants in problem-oriented discussion groups.
- Serve as a liaison with participants’ families/significant others/care providers.
Qualifications
- MS in social work from an accredited school of social work; or
- MA in psychology, gerontology or counseling from an accredited school and one year of experience in aging, health, or long-term services; or
- BA in social work from an accredited school with two years of experience in aging, health or long term care services
Preferred Skills
- Previous experience in ADHC a plus
- Bilingual English / Chinese ( fluent )
- Strong interpersonal skills
- Good command of computer and various software programs
