JOB SUMMARY:

The Congregate Housing Service Program (CHSP) Case Manager III is responsible for assisting seniors and adults with disabilities in overcoming barriers to maintaining housing and independent living. Case Manager III will rotate between several CHSP sites to provide the following services to participants: case management, advocacy, assessments, service coordination, health and wellness assessments.

 

ESSENTIAL JOB RESPONSIBILITIES:

  • Provides case management services, including development of a strength-based client relationship, comprehensive needs and strengths assessment, client referral and advocacy, crisis intervention, service coordination, development of outcome-based support plans and follow-up. Develops support plans and assists clients in a strength-based manner. Encourages and facilitates client participation and leadership in agency and community activities.
  • Implements services that are designed to improve service quality to seniors and adults with disabilities who reside in CHSP housing communities. Provides in-depth focus on hoarding disorder, medication management and nutrition. Recommends and assists in the implementation of program goals and objectives.
  • Completes accurate, legible and timely documentation of services delivered in accordance with Impact NW’s management information system. Ensures compliance with agency and funder documentation and evaluation requirements that support quality client services.
  • Conducts initial applicant screening assessments to assist in eligibility determination.
  • Serves as a liaison to program and collaborating organizations, including social service agencies and government and community groups.
  • Provides case and program consultation during scheduled staff meetings and the CHSP Professional Assessment Committee.
  • Serves as a liaison with partner organizations (such as Home Forward and Multnomah County Aging, Disability and Veterans Services Division and other service providers and contractors as necessary.
  • Effectively communicates agency policy and mission to collaborating organizations and the community.
  • Effectively works with individuals and provides support to people of all genders, races, ethnicities and backgrounds.
  • Meets client service standards set forth by Program Manager regarding quantity, type and quality of services to be delivered to clients.
  • Performs related duties and responsibilities as required or assigned by the Program Manager.

 

SUPERVISION:
Reports to: CHSP Program Manager

 

REQUIREMENTS AND QUALIFICATIONS

Any combination of experience and training that would likely meet the following requirements:

  • Bachelor’s degree or equivalent in social work, psychology, mental health or related field
  • Minimum of two years’ experience in human services working with seniors and/or younger adults with disabilities
  • Minimum of one year’s experience in mental health-related work
  • Ability to effectively work within a strength-based, integrated family support service delivery model
  • Ability to communicate effectively, both orally and writing
  • Demonstrated computer skills using Microsoft Office programs
  • Ability and willingness to work as a team member and support Impact NW’s mission and goals
  • Ability to work independently with remote supervision
  • Sensitivity and ability to relate to people from diverse lifestyles and cultures
  • Possess good organizational skills and ability to prioritize multiple and varied tasks

 

PREFERRED BUT NOT REQUIRED QUALIFICATIONS:

  • Graduate-level education and training in social work or related field
  • Bilingual and bicultural knowledge and skills

 

THIS JOB ALSO REQUIRES THE FOLLOWING PHYSICAL ACTIVITIES:

  • Repetitive motion, prolonged sitting and extensive visual involvement
  • Ability to efficiently travel in order to perform duties and other agency related errands
