RiverSpring Living is seeking a Transition Planning Manager to join our dynamic team!
The RiverSpring Living campus is set on 32 acres overlooking the Hudson River, in Riverdale, New York. RiverSpring Living provides a full range of care solutions including nursing home care, assisted living programs, senior housing, rehabilitation services and specialized services such as elder abuse prevention and memory care.
Position Overview
The Transition Planning Manager arranges necessary home care services and guides the entire care team in providing our patients with a safe transition back to the community and ensures thorough documentation of all activities and outcomes.
Additional responsibilities include providing patients and their families with emotional and social support, educating them about entitlement benefits, assisting with benefit applications and renewals, and supporting residents in maintaining their benefits throughout their rehabilitation stay.
Qualifications
- Master’s degree in social work (MSW)
- Preferred experience in geriatrics and discharge planning with a minimum of two years’ experience and/or training in assessing, case management and providing excellent customer services to this population
Compensation
- Annual Salary: $66,300 - $72,000
