Family Support Worker
Position Summary
This position provides direct service through managing a caseload of 20-25 participants, coaching them to achieve their parenting and child development goals while helping eliminate barriers.
Essential Functions
Maintain a caseload of 20-25 participants and assist them in achieving their parenting and child development goals while removing barriers. Maintain a minimum of twice a month in person contacts with the participants for an average of 44 appointments per month lasting 25 minutes minimum and document case notes. Record participant progress towards goals in all required databases and program documents within 7 days of contact. Conduct screenings and assessments based on program requirements. Provide internal and external referral services. Maintain working knowledge of SMCS programs to successfully navigate connections through the family-centered care model and provide outreach to community and families. Adhere to agency policy, procedures, and the professional code of ethics. Participate in relevant training opportunities for professional growth and development. Participate and contribute to regular staff meetings and supervisory sessions. Satisfactorily complete any other special assignments as requested by the supervisor.

Evidence of high levels of motivation, initiative, perseverance, and independent judgment. Ability to work in a variety of settings with culturally diverse families and communities. Proven ability to establish and maintain ongoing professional relationships with program participants, team members, and partner agencies. The ability to support individuals and families through crises toward self-sufficiency. Ability to motivate others to achieve their goals, experience with strength-based assessment, the family coaching model, and trauma informed approach. A strong sense of respect for confidentiality involving participants and fellow employees. Excellent technology skills, knowledge and proficiency in Microsoft Office. Ability to troubleshoot and solve problems logically and methodically. Excellent organizational skills and attention to detail. Ability to work well independently or in a group and excellent time management skills. Strong teamwork and interpersonal skills.

Education and Minimum Qualifications
One-year related service experience. Proficiency in Microsoft Office Suite.
Preferred Experience and Qualifications
Bachelor’s degree in social work, Early Childhood, or a related field. Spanish fluency encouraged, but not required
