Care Coordinator – Women's Residential Recovery Services

At ANEW, we support women and families in recovery through integrated chemical health, mental health, and family-centered services. As a Care Coordinator, you will play a critical role in bridging clinical care, medical services, and client advocacy—ensuring our clients receive safe, coordinated, and compassionate care every step of the way.
What You’ll Do –
Medication Management & Clinical Support
- Administer medications in accordance with established protocols and regulatory standards
- Maintain accurate, compliant medication records (QuickMAR)
- Monitor for and escalate any medication-related concerns
- Manage medication inventory and ensure proper storage and tracking
- Review after-visit summaries and implement new medical orders
Care Coordination & Medical Scheduling
- Coordinate and schedule medical appointments (primary care, specialty, follow-ups, etc.)
- Communicate directly with healthcare providers to ensure continuity of care
- Arrange and coordinate transportation for client medical needs
Client Advocacy & Support
- Support clients in navigating the healthcare system with clarity and confidence
- Advocate for client needs across providers and systems
- Collaborate with clinical and interdisciplinary teams to support individualized care plans
Program Operations & Collaboration
- Complete health intake assessments for new admissions
- Approve and track medical passes
- Partner with pharmacies and ANEW medical providers to ensure seamless care delivery
- Train and onboard staff on medication administration and documentation systems
Professional Accountability
- Maintain all required trainings and certifications
- Stay current on best practices in behavioral health and medication administration
