Care Coordinator

In this role, you will support individuals in their recovery and mental health journeys by connecting them to housing, employment, and community resources. You will build meaningful relationships with clients, provide ongoing support, and collaborate with local agencies and service providers to help individuals achieve personal goals and long-term stability.

Qualified candidates should have a degree in Social Work, Psychology, Human Services, or a related field (or be nearing graduation), along with strong communication skills, organization, and a passion for helping others.
