Clinical Care Coordinator

In the Clinical Care Coordinator position, you will deliver integrated behavioral health support in pediatric outpatient settings, serving children ages 0–17 and their families. In this role you will collaborate with medical and behavioral health teams to coordinate care, conduct screenings, connect families to community resources, and support early intervention through the Collaborative Care Model and telehealth, with a focus on improving access and outcomes in underserved communities. 

 

Essential Functions

A) Provide behavioral health consultation within assigned pediatric primary care clinics using a collaborative care approach, partnering with primary care providers, behavioral health specialists, and care managers to ensure coordinated, patientcentered care.

B) Coordinate telehealth consultations between child and adolescent psychiatrists, primary care providers, and school staff, and develop and maintain secure, efficient teleconsultation workflows and protocols.

C) Educate and support pediatric providers in the use of mental health screening tools, workflows, and evidencebased practices.

D) Deliver education and training to school personnel on child and adolescent mental health, including early identification, common signs and symptoms, and referral pathways.

E) Coordinate telehealth consultations between child and adolescent psychiatrists, primary care providers, and school staff, and develop, implement, and maintain secure and efficient teleconsultation protocols and workflows.

F) Coordinate and manage referrals among schools, pediatric clinics, emergency departments, community providers, and behavioral health systems, including tracking outcomes and conducting followup to support continuity of care.

G) Serve as a liaison between families, community organizations, and behavioral health systems to support navigation, access to services, and collaboration across partners.

H) Provide psychoeducation to families regarding mental health conditions, treatment options, and care plans, and support caregiver understanding and active participation in their child’s behavioral health care.

I) Connect families to community resources and basic needs supports, including housing, food assistance, transportation, and other social determinants of health services.

J) Participate in data collection, outcome tracking, and reporting to support program evaluation, quality improvement initiatives, and performance monitoring.

K) Support the integration of traumawise care principles, including felt safety, connection, and regulation across pediatric healthcare settings, schools, and daily practice to support patients and staff.

L) Promote and strengthen TrustBased Relational Intervention (TBRI) practices in collaboration with community partners, patients, families, and employees.

M) Serve as a communication liaison between patients, families, and prescribers, including accurate and timely documentation of patient interactions and clinical information.

Minimum qualifications: 

Minimum Education: Master’s Degree 

Defined Education: Counseling, Social Work, or related Health and Human Services field 

Preferences: Spanish-speaking 

Substitutions: None 

Years’ Experience: One (1) year 

Defined Experience: Health and Human Services, mental health, or behavioral health-related field 

License/Certifications:  

For Master’s Degree in Counseling related field: intern status license as Licensed Professional Counselor-Associate (LPC-A), Licensed Marriage and Family Therapist-Associate (LMFT-A), and/or Licensed Master Social Worker (LMSW) in the state of Texas and actively working on obtaining professional license as LPC, LMFT, or LCSW (obtaining practice hours and required examinations); or fully licensed as LPC, LCSW or LMF; Valid Texas Driver’s License
