The Greater Community AIDS Project (GCAP) is seeking a compassionate Case Manager to help individuals living with HIV/AIDS achieve housing stability and improved quality of life in East Central Illinois.

Key Responsibilities:
- Manage client transitions into permanent supportive housing.
- Advocate for clients and connect them to healthcare, employment, and social service resources.
- Intensive case management using motivational interviewing and strengths-based approaches
Requirements:
- Bachelor’s degree in social work, Human Services, OR a related field with equivalent hours of human services coursework. Candidates currently pursuing a degree with relevant coursework are also encouraged to apply.
- Experience in motivational interviewing, trauma-informed care, and crisis de-escalation is preferred
- Valid driver’s license and reliable transportation (must be willing to transport clients).
