Part-Time Respite/Personal Support Provider
Community Horizons – In-Person Position

A warm, Christian family partnered with Community Horizons is seeking a caring, patient, and values‑driven support provider to spend time with four wonderful kids—two boys (age 10) and two girls (ages 8 and 10). Their home is full of kindness, respect, and healthy boundaries, and they’re excited to welcome someone who shares those values.
This is a pet‑friendly home.

  • Consistent after‑school hours

  • 3:30 PM – 7:30 PM, three days a week

What You’ll Be Doing

  • Checking in with parents to plan your shift
  • Helping with homework and school routines
  • Guiding the kids through evening and bedtime routines
  • Enjoying fun, age‑appropriate activities:
  • Boys: games, Legos, cars, creative play
  • Girls: conversation, games, homework help, and bedtime routines

This role is perfect for someone who loves being hands‑on, playful, and encouraging.

What We’re Looking For

  • Able to pass a background check
  • Reliable transportation
  • Kind, patient, friendly, and aligned with Christian values
  • Experience supporting children or adults with behavioral challenges
  • Experience with autism
  • Experience with children who have been adopted or spent time in out‑of‑home placements

Community Horizons offers support for individuals enrolled in self-directed programs through managed care organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of third-party fiscal agents responsible for payroll.
