SUMMARY
The Coordinated Family Support (CFS) Coordinator aids adult individuals with intellectual disabilities to continue living in their family home by arranging and facilitating necessary services and support systems. Coordinators will be expected to provide access to these programs, services, and resources within the individual’s home and the surrounding community. These supports are intended to help individuals better access their community and develop skills for more comfortable home living.
ESSENTIAL DUTIES
- Scheduling and notifying individuals and/or their caretakers of upcoming visits
- Visits the home of the individual and their family and/or caretakers to evaluate their needs and goals
- Assesses the routines and capabilities of the individual, their caretaker, family, and home
- Provides information and assistance to the individual and their family in navigating available services and programs
- Prepares and records the needs of the individual using dedicated report forms
- Research and source possible services, programs, and generic resources to further the individual’s goals and needs
- Communicates possible plans and support services available to families
- Assists with scheduling services & program deliveries for individuals, including medical or other appointments
- Facilitates communication and service coordination between the home and community agencies
- Maintains contact with the individual, service providers, and other parties throughout the fulfilment process
- Manages, updates, and submits reports on the progress of service or program fulfillment
- Strictly maintains and adheres to the confidentiality of all information about clients, families, and employees
- Corresponds regularly with other ARK Team members regarding the plans and service fulfillment of individuals
QUALIFICATIONS
- Must be at least 18 years of age
- Pursuing a Bachelor's or Associate's level degree in Human Services, Social Work, Psychology, Sociology, or a related field of study, OR have 1 year of experience in the developmental disabilities services
- Bilingual with the ability to effectively read, write, and communicate in English and Spanish is required.
- Comfortable working with & explaining complex details to individuals with intellectual disabilities
- Proficient in MS Office and Google Suite Applications
- Have driver's license and reliable mode of transportation
Additional Competencies
- Experience using JOT
- Customer service experience
- CPR, First Aid training, or other BLS certifications
