Make an Impact with Adolescents and Families

Children’s Home of Reading (CHOR) – Germantown STLP is seeking a compassionate Case Manager to provide therapeutic case management services to adolescents and their families. This role is ideal for professionals who thrive in collaborative environments, advocate for youth, and coordinate care across multiple systems.

As a Case Manager, you will maintain residents’ Individual Service Plans (ISP) and medical records, coordinate appointments and in-home visits, and track all visitation, ensuring compliance with all licensing and regulatory standards.

đź’Ľ Key Responsibilities

Medical Responsibilities

Maintain residents’ medical records and ensure compliance with all regulatory standards

Track and schedule all routine, specialist, follow-up, and psychiatric appointments

Assist in medical appointment transportation as needed

Monitor and administer prescribed medications; complete weekly medication counts

Train staff in medication administration, CPR, and first aid

Communicate medical updates to parents, legal guardians, and relevant stakeholders

Participate in medical-related staff trainings and provide instruction when needed

Individual Service Plan (ISP) Responsibilities

Maintain ISPs, ensuring compliance with all licensing and regulatory standards

Schedule and track ISP submissions, goals, and summaries in the Electronic Health Record (EHR)

Notify involved parties of ISP meetings and document notifications in EHR

Support behavioral goals and mentor direct care staff in goal creation and adjustment

Attend and assist in facilitating ISP meetings

General Responsibilities

Fill in for Youth Behavioral Specialists and Supervisors as needed

Perform additional duties as assigned

Education/Experience

Bachelor’s degree required

Strong written and verbal communication skills

Ability to read, comprehend, and professionally document information

Critical thinking and problem-solving abilities in complex situations

40 hours of annual training after initial year (30 hours initial year)
