Physical Therapist (Full-Time | Monday–Friday | 8:00 AM–5:00 PM)

What You’ll Do as a Physical Therapist:

Deliver hands-on, patient-centered care in the comfort of patients’ homes

Evaluate functional needs and create personalized plans of care

Track progress, adjust treatment plans, and celebrate patient wins

Collaborate with physicians, caregivers, and interdisciplinary teams

Mentor and guide therapy assistants and support staff

Complete timely, accurate documentation to support excellent outcomes

Act as a clinical ambassador for physical therapy in the community

 

This role is active and engaging, requiring mobility, lifting, and patient transfers—but the reward is seeing real progress where it matters most.

 

What We’re Looking For:

Graduate of an APTA-approved Physical Therapy program

Active, unrestricted Physical Therapist license in the state of practice

At least one year of experience in a healthcare setting

Willingness to travel within the assigned coverage area (up to 50%)

Reliable transportation, valid driver’s license, and current auto insurance

Ability to lift 50–100 lbs and perform essential physical functions of the role

 

Reporting Structure:

You’ll report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee and be supported every step of the way.
