Case Manager - Permanent Supportive Housing Services 

Full Time (40 hrs./wk.) Exempt

 

GENERAL SUMMARY:

The Case Manager will provide case management, advocacy, and support services to residents living in Permanent Supportive Housing. The role focus on maintaining housing stability, increasing self- sufficiency, and enhancing overall well-being through person—centered, trauma-informed care.

 

JOB RESPONSIBILITIES:

  • Develop, implement, and monitor individualized service plans in collaboration with residents.
  • Conduct regular home visits and wellness checks to assess client needs and safety.
  • Coordinate access to mental health care, substance use treatment, primary care,  employment support, and other community services.
  • Assist residents in navigating housing retention challenges, including lease compliance, conflict resolution and budgeting.
  • Maintain accurate and timely case notes, assessments, and service documentation in compliance with agency and funding requirements (e.g. HUD, HMIS).
  • Advocate on behalf of residents with landlords, service providers, and other  stakeholders.
  • Participate in multidisciplinary team meetings and case conferences
  • Provide crisis intervention and de-escalation support as needed
  • Maintain confidentiality and uphold ethical standards.
  • Support the mission and values of Cass Community Social Services in all  interactions and duties

JOB REQUIREMENTS:

  • Bachelor's degree in Social Work or related human services field required.
  • State licensure (LLBSW, LBSW, LLMSW, or LMSW) preferred.
  • Minimum 1-2 years of experience working with unhoused individuals, mental illness, or substance use disorders or equivalent education.
  • Strong understanding of housing-first principles, harm reduction, and trauma-informed care.
  • Knowledge of community resources and support systems.
  • Excellent communication , computer, organizational, and documentation skills
