Street Outreach Navigator

A PATH (Projects for Assistance in Transition from Homelessness) Street Outreach Navigator position involves engaging with unsheltered individuals who have mental health conditions, building rapport, and providing case management to help them access services and secure permanent housing. 

Key duties include street-based outreach, crisis intervention, assessing needs, developing service plans, and assisting clients with applications and referrals to services such as mental health care and housing. 

The role also requires maintaining detailed client records and collaborating with partner agencies, law enforcement, and other community organizations.
