🛠️ What You’ll Do:

• Evaluate patients to determine rehab needs and develop individualized care plans.
• Implement and update Plans of Care to ensure quality and continuity.
• Conduct therapy visits and reassessments per OASIS and PPS guidelines.
• Communicate changes in condition to physicians and care teams.
• Educate patients and caregivers on daily living activities and therapy programs.
• Supervise COTAs, HHAs, and clinical staff to ensure care plan adherence.
• Document all care and progress in accordance with agency standards.

✅ What You’ll Need:

• Graduate of an accredited Occupational Therapy program
• Current, unrestricted OT license (IL)
• CPR certification (AHA or Red Cross)
• 1+ year of clinical experience
• Reliable transportation, valid driver’s license, and auto insurance
• Ability to travel up to 50% within the territory
• Physical ability to lift 50–100 lbs and perform full range of motion

The low end of the compensation range is 90000 to the high end up to 100000 annually, which can include additional earnings for work beyond quota, bonuses, on-call, or other performance-based compensation variables.
